Your agreement to the following clinic policies is required for you/your child to receive professional services.
Confidentiality
There is no guarantee of confidentiality under the following conditions:
- If we suspect you/your child are/is in imminent danger of harm to self or others, or a child or elderly person is being abused or neglected (as we are mandated reporters)
- If a court orders a release of information
- If you initiate a malpractice lawsuit, or a billing dispute with a financial institution
- If your insurance company requests to review your/your child’s case
- If you pay by credit card, our name will appear on your credit card statement
- If you do not pay your bill, your balance due statement (including diagnostic and procedural codes) may be sent to a collections agency or other responsible party
- Between our administrative staff, or colleagues with whom we consult professionally
- You consent to be contacted by email regarding Nema Health services that may be of interest to you in the future.
- You consent to be contacted by email or EMR patient portal for up to one (1) year after your Clinical Evaluation Session in order to determine symptom stability.
Payment
SELF-PAY RATES
If you are paying for Nema Health out-of-pocket, you agree to the pay the fees outlined below:
- Clinical Evaluation (75 min):Â $400
- Re-Evaluation Session (75 min): $300
- Therapy Sessions, including Care Session, Follow-Up Session, Mental Health Check-In (50 min): $275
- Peer Mentor Session (30 min): $50*
- Support System Session (30 min): $50
- Peer Group Session (60 min): $25
- Clinical Group Session, including Graduate Group, Skills Group, DBT Skills Studio (60min): $50
- Physician (Medication) Evaluation (60 min): $300
- Physician Visit (Medication Management) (25 min): $175
*2 Peer Mentor Sessions are included free of charge (1 during intensive care and 1 at the beginning of Rise); any additional Peer Mentor Sessions are $50/session.
If you would like assistance with out-of-network reimbursements in the form of a superbill, please email billing@nemahealth.com or message Nema Health Care Coordinator in the patient portal.Â
Please note that medication management services are an additional cost. The provider who completes your Clinical Evaluation will discuss whether medication management is appropriate for you at your Clinical Evaluation.

INSURANCEÂ COVERAGE
If we've verified your insurance eligibility and your plan is in-network with Nema Health's services, the above fees do not apply. You may still be subject to co-pays, co-insurance, and deductibles, depending on your individual plan's coverage. These fees will be processed after the claim has been fully finalized by the insurance provider.

LATEÂ CANCELLATIONSÂ &Â NOÂ SHOWS
We are committed to providing you with the highest quality of care. To help ensure that all patients have access to our therapy services, we have a cancellation policy that reflects the importance of scheduled appointments.
If you need to cancel or reschedule with your Nema Health care team, kindly give 24 hours notice. This allows us to offer that time slot to another patient who may be waiting for an appointment. Please reach out by phone, email, or secure message and the Nema care team will reach out to assist with cancellation or reschedule.
No shows or cancellations made less than 24 hours prior to the start of an appointment will incur the full session costs outlined above. Please give your calendar a thorough look before scheduling and reserve rescheduling for emergencies only. We appreciate your understanding and value that you have chosen to pursue healing with us.

Additional Fees
- Late Balances: There is a $50 late fee for balances more than 30 days past due, and they may be submitted to collections after 30 days, along with any associated collections fees.
- Re-Evaluation Fee: If for some reason you do not complete treatment with Nema Health (e.g. hospitalization) and then decide you would like to return to Nema Health, we require a re-evaluation to determine whether it is appropriate for you to return. We will also make this decision in conjunction with the appropriate collateral providers you are involved with. The cost of this re-evaluation is $300.
- Legal Fees: If you require that Nema Health or any of its employees participate in any investigation, arbitration, litigation, settlement, mediation, suit, action, or other legal proceeding you will be billed for any time expended on complying with the request at a fee of $350 per hour. You will also be responsible for any legal fees that Nema Health incurs as a result of any required participation.

You are financially responsible for all charges, whether or not:
- Insurance pays for any services
- Treatment is successful, for which there cannot be any guarantee.

If you are unable to comply with payment obligations (i.e., non-payment of invoices), Nema may need to terminate services. Prior to the decision, we will make every reasonable effort to notify you of the billing issue and work with you to resolve it. If the issue is unable to be resolved, we encourage you to seek alternative care. Nema will provide referral information at your request.
